Resident Team Conferences
RTC’s are held about twice a year for each resident to provide an opportunity for many of the team members mentioned above, the family, and the resident if appropriate to get together and review how the plan of care for the resident is working from everyone’s perspective. The conferences are about 15 minutes in length and are a more formal way of connecting everyone. Families are encouraged to dialogue regularly with any staff member they wish about day to day life and concerns and the RTC’s provide a review of those processes tp ensure planning together is working
